Collection Statement – Patients

The Everyday Support Program is a complimentary service provided by UCB Australia’s Pty Ltd (“UCB”) (ABN 48 005 799 208) authorised agents of Level 1, 1155 Malvern Road, Malvern VIC 3144. This Collection Statement explains how your personal information is handled in connection with your enrolment and participation in the Everyday Support Program (“Program”). The Program provides support for patients who have been prescribed Cimzia® (certolizumab pegol). If you would like to participate, please take a few moments to read this statement. By completing the form, you agree to its terms. The Program is made up of injection training, an emotional support program and an educational website. The injection training and emotional support program are administered by Zest Healthcare Communications (ABN 81 115 828 941), Suite 2, Level 9, 1 Elizabeth Plaza, North Sydney NSW 2060 (“Zest”), on behalf of the sponsor, UCB. UCB and or its nominated agents administer the educational website. Zest collect and process your information including health and sensitive information (‘Personal Information’) required for your participation in the Program. If you agree to participate in the Program, Zest will collect further Personal Information from you during the course of the Program. The Personal Information collected will include: your and/or your carer’s contact details, your doctor, your preferred appointment venue, your relevant medical history, competency with injection technique and process, as well as other Personal Information necessary for the Program. Zest collects and holds your information for various reasons (as more fully described in the Privacy Policy referred to below) including to fulfil the administrative requirements of programs, for communication with you regarding these programs and for the analysis, ongoing improvement and development of our programs and services. Your personal and sensitive information may be disclosed to service providers who assist us in administering the program, and to your healthcare team (such as a doctor, nurse, pharmacist or hospital). Some of the third parties to whom we disclose personal information may be located outside the state or territory in which the information was collected or outside Australia. The state/territories and countries in which such third parties are located will depend on the circumstances. For example, we may disclose personal information to our related companies overseas and to our overseas service providers. By providing the information requested, you consent to Zest using this information for these purposes. Generally, you have no obligation to provide any personal information requested by us. However, if you choose to withhold requested personal information, we may not be able to provide you with the services that depend on the collection of that information. In case of an adverse event being reported whilst enrolled in the Program, Zest are legally obliged to pass on to UCB, as the sponsor of Cimzia®, details of any adverse events involving its products that they become aware of. Only de-identified data, meaning data not disclosing your identity, will be provided to and used for this purpose by UCB. UCB or an authorised third party may contact your healthcare professionals for the purpose of adverse event follow-up. Your personal information will be kept securely in accordance with the current Australian Privacy Principles contained within the Australian Privacy Act (1988).
